FIRST VISIT INFO
During your first visit to Maxillo Tandem Maxillofacial Surgery Center you will meet with the surgeon to discuss your needs and answer any questions you might have. Depending on your condition, you might be offered various treatment options. If a surgical intervention is required, this procedure will take place at your next visit.
PRIOR TO YOUR FIRST VISIT
Please make sure you have completed the medical questionnaire. Bring any x-rays, written reference forms or any other document that might pertain to your visit.
X-RAYS
It is your responsibility to supply us your most recent x-rays prior to your first visit or to have them with you during the consultation. If this is not the case, x-rays will have to be taken and fees will be applied.
METHODS OF PAYMENT AND FEES
When booking your first consultation, we will inform you of the fees associated with this visit. Fees must be paid at the end of each visit by debit, credit or cash payment. If you are covered by private medical insurance, we will supply you the required forms for your claim. Following your first consultation, you will be given a treatment plan and an estimate to submit a predetermination to your insurer.
